Applications Being Accepted for Initiative to Improve Health and Reentry Outcomes

In partnership with the California Health Care Foundation (CHCF) and LA Care, Transitions Clinic Network (TCN) is currently accepting applications from primary care clinics throughout California interested in implementing the TCN program. Up to 25 sites will be selected to participate in this year-long initiative to improve the health and reentry outcomes of individuals with chronic health conditions returning to the community from incarceration.

The Transitions Clinic Network program is an evidenced-based model of care that reduces ED visits, hospitalizations and parole and probation violations for patients impacted by the criminal justice system. This is a unique opportunity for clinics to play a role in not just improving patients’ health, but improving communities’ health by addressing public safety and reducing mass incarceration.

Only sites that submit a complete site assessment by Friday, November 16th will be selected to submit a full proposal.

Key Dates

Application/Site Assessment Due: Friday, November 16, 2018,at 5:00 pm.

Sites Selected to Submit Full Proposal By: Tuesday, November 27, 2018.

Full Proposals Due: Friday, December 14, 2018 at 5:00 pm.

Kick Off Meeting: Tuesday, January 29, 2019.
