Second Round Whole Person Care Pilots Approved

The second round of Whole Person Care Pilot Projects was just announced by the Centers for Medicare and Medicaid Services. The goal of the Whole Person Care Pilots is to expand efforts to provide comprehensive health care to medically-complex patients. Of the 15 pilots approved in the second round, eight of the pilots build upon previously-approved pilots.

Whole Person Care is designed to target Medi-Cal patients with complex medical issues and frequent users of multiple health systems. These pilot projects coordinate physical health care, behavioral health care, and social services to improve the health and well-being of beneficiaries through an effective use of resources. Specific target populations of approved pilots include individuals recently released from institutions or incarceration, those with mental illness or substance use disorders, and those who are currently homeless or at risk of homelessness.
