The Department of Health Care Services (DHCS) will open the
30-day application window for the Providing Access and
Transforming Health (PATH) Capacity and Infrastructure
Transition, Expansion, and Development (CITED) Round 3 on
January 15, 2024.
PATH CITED provides funding to build capacity
and infrastructure for the delivery of Medi-Cal enhanced care
management (ECM) benefits and community support services.
Eligible applicants include counties and other local government
agencies, community-based organizations, federally qualified
health centers, providers, and others as approved by DHCS. The
deadline to apply for this round of funding is February
15, 2024.
DHCS published an
application outline for PATH CITED Round 3 to assist
prospective applicants prepare the necessary information required
to complete the application. Although the application outline is
not the final approved format for applying for PATH CITED Round
3, applicants are encouraged to use the outline as a planning
tool prior to application release on January 15.
