PATH CITED Round 2 Funding Awards Announced

This week, the Department of Health Care Services (DHCS) announced $144 million in awards to 145 Enhanced Care Management (ECM) and Community Supports organizations and providers across the state as part of the Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion, and Development (CITED) initiative. These awards come from Round 2 funding of the PATH CITED initiative, which is intended to enable the transition, expansion, and development of ECM and Community Supports capacity and infrastructure.  A full list of the 145 awardees, which includes county departments and agencies, can be found here.

ECM and Community Supports serve the highest-need Medi-Cal managed care members in their communities through community-based organizations and providers. The awards announcement this week will be used to support investments in provider workforce, infrastructure and IT systems, and capacity building to extend health care into communities, increase coordination of care, and provide in-person care management to members in their communities.

The application window for PATH CITED Round 3 is expected to open in early 2024. Additional information about the initiative can be found on the PATH website.
