CA Rural Health Transformation Funding Opportunities, Advisory Council Membership Announced

The California Rural Health Transformation (CalRHT) program is California’s approach to the federal Rural Health Transformation Program (RHTP), a $50 billion investment over five years, available to all 50 states. Through RHTP, California has been awarded $233.6 million for Federal Fiscal Year 2026 to support rural communities across the state. The Department of Health Care Access and Information (HCAI) is leading the CalRHT program with oversight from the Centers for Medicare & Medicaid Services (CMS). 

The CalRHT program contains three initiatives, each with their own set of programs: Transformative Care Model, Workforce Development, and Technology & Tools. More in-depth information about these three initiatives can be found in the CalRHT Project Narrative and Budget Narrative.   

The CalRHT program has released additional information about grant opportunities to strengthen health care delivery in rural communities where workforce shortages, infrastructure gaps, and accessibility create unique barriers to care. 

CalRHT is opening three grant funding opportunities in July: 

  • Accelerator Partners – supports collaborative models that seek to strengthen primary and maternity care across rural communities. The Grant Guide is now open, and the application portal is expected to open by mid-July.  
  • Electronic Health Record (EHR) Modernization – supports technology systems, data exchange, and digital tools that enable coordinated, efficient care delivery across rural networks. The grant guide will be available mid-July, with applications to follow.  
  • Workforce Development, Recruitment, and Relocation – aims to strengthen and sustain the rural healthcare workforce by improving recruitment, retention, and training for health professionals. The grant guide and applications are anticipated to be available before July 31.

The Rural Health Policy Council (RHPC) is an advisory group whose role is to provide expertise and feedback to HCAI on the design and implementation of the CalRHT program and general issues affecting health in rural communities. The membership, which was recently announced, includes a broad range of stakeholders, including rural health facilities, providers, and consumers. Notably, Calaveras County Public Health Director Colleen Rodriguez was selected to represent county public health perspectives on the Council.  

The RHPC meets quarterly, with meetings open to the public. The next upcoming meeting will be on Tuesday, July 28, from 10:00 am to 12:00 pm. Advance registration is required. Additional information on the RPHC can be found on the CalRHT website.
