A Spotlight on Counties’ Best Practices

April is National County Government Month. During the month, CSAC is producing a series of videos and blog postings highlighting California Counties’ best practices. The programs we are spotlighting are recipients of our annual Challenge Awards, which recognize the innovative and creative spirit of California county governments as they find new and effective ways of providing programs and services to their citizens. The Challenge Awards provide California’s 58 counties an opportunity to share their best practices with counties around the state and nation.The programs being highlighted through these videos are recipients of the 2013 awards. The Call for Entries for the 2014 CSAC Challenge Awards has been distributed; the entry deadline is June 27, 2014.

Mono County’s “Whole Person Wellness Approach”

There’s historically been a pioneer spirit in Mono County. That spirit is alive and well to this day; you see it as you travel along Highway 395. You hear it when you stop and talk to the residents of communities such as Bridgeport and Lee Vining. That pioneer spirit is also alive and well in the Mono County Behavioral Health Department, where the staff is urged to be creative and look for new ways to engage clients and provide services.

The result has been the creation of a “Whole Person Wellness Approach” that looks at the totality of the client – not just the mental health side. It started by asking a consumer on each visit if he or she wanted a blood pressure check; from there, staff began tracking weight and surveying consumers on their perceived wellness. Soon, staff was having conversations with clients in a manner they never had before. Both staff and clients began to see the bigger health picture.

Nevada County’s System of Care Approach

“One barrier you need to take off the table is “well, we don’t have money to do it.” It doesn’t take any money to sit down at a table and talk with people. That’s a freebie, with a huge payoff.” With those words, Nevada County Health Director Michael Heggarty sums up their approach to providing a system of care for kids, teens, and their families. They have broken down silos, ignored the barriers and are working across several different organizations to provide a network of services that starts with mental health, but has grown into much, much more.

To be honest, it started with a cluster of teen suicides in Nevada County—tragedies that while not statistically out of the norm, hit very close together and very close to home. They made people realize they needed to do more to reach out to kids and their families. And so even as funding for collaboration dried up, they collaborated.  There is a monthly meeting that includes the leaders of the County Health Department, the Superintendent of Schools, the Courts, Probation, Child Welfare and others agencies that have contact with kids and their families. That trickles down to weekly meetings among other staffers.

Ventura County’s New Crime-Fighting Tool — iCop

In the inner sanctum of the Ventura County Sheriff’s Offices, there’s a display case that houses numerous department artifacts dating back to the 1800s: photographs, badges and handwritten incident logs help tell the department’s storied past. Captain Chris Lathrop likes to stop and take in the department’s rich history. But Captain Lathrop’s focus is in the future – and one mobile application in particular that is making a big difference in how the department fights crime.

The mobile app developed in-house by Ventura County’s IT Department is called iCop – and it’s been a great asset to Sheriff’s Department deputies and investigators.

Believed to be the first of its kind in the nation, this mobile application interfaces with the County’s criminal justice system, allowing officers to securely search by person, address or vehicle. Results include demographics, mug shots, tattoo images, warrant information, booking history, calls for services, arrest reports and property reports. Information is returned to the field in a matter of 20 seconds or less — as compared to the hours or days it previously took.

Riverside County Reduces SSI, Increases Patient Safety

When Riverside County Medical Center staff compared its rate of surgical site infections (SSI) to other hospitals around the country, they realized they had an issue. The hospital’s ratio of infections was more than 200 percent above the recognized baseline. Something had to be done; something was done. And the result has been an amazing turnaround for the Medical Center that has resulted in improved patient care, heightened patient safety and a reduction in medical care costs.

To combat this high infection rate, the hospital formed a team consisting of surgeons, anesthesiologists, OR nurses, infection preventionists, housekeeping personnel and researchers. The team developed protocol for staff and education plan for patients to follow. Beyond following new protocols, staff found themselves participating in a cultural change.

Stanislaus County’s Clean and Sober Living Program

The county programs CSAC recognizes with our Challenge Awards all have one thing in common: they represent the best practices counties put to use every day serving the people of their community. The award given to Stanislaus County is a little different. It recognizes a truly great program—and also the extraordinary effort made to save the program in the face of severe budget cuts. This is the story of the Stanislaus County Clean and Sober living program.

The program is designed to let mothers who are going through alcohol and substance abuse treatment live with their children in a safe and supportive environment. Keeping these families together is usually far better for the children, and the prospect of keeping their kids often provides moms with the extra incentive they need to complete the program successfully. It saves money for the County because the alternative is often foster care for the kids and incarceration for the moms.

San Bernardino County’s West Nile Virus Aerial Surveillance Collaboration

Most of us have swatted away marauding mosquitos, attacking from the sky—well now the mosquitos have to watch out for the air power too. In San Bernardino County, the Mosquito Vector Control Program is collaborating with the County’s Sheriff’s Aviation Division, and using state-of-the-art technology to identify potential mosquito breeding sites: unmaintained swimming pools.

In 2012, West Nile Virus – transmitted by mosquitos – was being reported at record levels. Dozens of cases occurred in San Bernardino County, where concerns were elevated over the fact that more than 18,000 homes in the County had been foreclosed.  Many of these residences included unmaintained swimming pools, ideal breeding grounds for mosquitos.

I admit, there are times when I wish it was a little simpler to use my smart phone to just make a phone call. But for every time I struggle to find my way back to the keyboard, there are at least five times when I am thankful for this app, or that function that makes my life easier. What was the name of that restaurant again? Is it on 9th or 10th street? Where is the nearest gas station? Smart phones are supposed to make our lives easier with that type of information at our fingertips, and now thanks to the Sacramento County Registrar of Voters, your smart phone can even make it easier for you to participate as a voter!

Sacramento County Registrar of Voters Jill LaVine challenged her staff to develop a mobile app for voters. But her I-T staff had never developed an app before. They took some classes on their own time, learned some new skills and began putting them to use. The result is the SacVote App for IPhones and Androids.

Riverside County’s Prevention and Early Intervention Mobile Services

Clinical therapists are taking their jobs a few steps above and beyond the typical call of duty in Riverside County. In fact, they are taking them 39 feet beyond.  That’s the length of each of the three mobile clinics that the therapists themselves drive out to different regions of Riverside County. It’s all part of the County’s highly successful Prevention and Early Intervention Mobile Services program.

These mobile clinics, which are specially designed recreational vehicles, bring mental health services to families with children up to 7 years old.  The program meets a need to develop services outside the standard clinic model,  according to Emma Girard, Senior Clinical Psychologist for Riverside County, who helps lead the project.

Sonoma County’s Juvenile Hall Partnership with the Boys and Girls Club

Kids who end up in Juvenile Hall have done something illegal. Why should their stay there be anything other than punishment? As the head of Sonoma County’s Juvenile Hall told me, “The kids in here today are going to get out, and they are going to be your neighbors, and your kid’s classmates.” Punishment is part of it, but so is rehabilitation. That’s why Sonoma County Juvenile Hall has formed a unique partnership with the Boys and Girls Club in their community. 

“The Club” took over some space inside Juvenile Hall, installed some games, computers, and lightened up the institutional look with paint, posters and some comfortable furniture. They provide activities, fun, and a safe environment for the kids to play, to learn, and to be just kids for a while, instead of kids in “Juvi.” But the most important thing “the Club” provides is incentive. The Club is the carrot at the end of the stick.

San Bernardino County’s Making Attendance a Priority Program

Truancy is a major issue in many communities — and San Bernardino is no exception.  Truancy can also have significant impacts beyond hindering education. According to the Office of Juvenile Justice and Delinquency Prevention, “Dropouts are poorly prepared to enter the workforce and require greater expenditures for social services and criminal justice process than do graduates.”

The San Bernardino County Public Defender’s Office was seeing the impacts of truancy first-hand. “Truancy is a big indictor on whether or not kids are going to continue in the juvenile justice system or are able to break out and get on with their lives, explained Assistant Public Defender Chris Gardner.” We had been looking for a way to get more involved, to partner with the schools to work on the truancy issue.”

Tulare County’s Teen Digital Media Lab

When Faythe Arredono talks about Tulare County’s Teen Digital Media Lab, her eyes light up. That sparkle is only matched by the teenagers who participate in the program. Ask the likes of students Araceli Mendoza or Vincent Macareno and you can see why this program has such a positive impact.

Tulare County library staff knew they could do a better job of connecting teens through the library, but the question was how. “We tried some things. Some worked, some didn’t, but we could never really get anything off the ground,” explained County Librarian Jeff Scott. That’s where Faythe Arredondo comes in. She was hired in 2011 as the County’s first-ever Teen Services Librarian. Right away she started exploring ways to get teens engaged digitally. With the help of a grant from the California State Library, she was off and running.

Tehama County’s AB 109 Auto Shop

Everyone likes a “Win-Win” situation—a solution to a problem that meets everyone’s needs. In Tehama County, they had a problem, and the solution they came up with has been a “win” on several levels. The AB 109 Auto Shop won a 2013 CSAC Challenge Award because it is reducing the jail population, training inmates, saving money for the Sheriff’s Department AND reducing costs and downtime for other county departments when their vehicles need maintenance and repairs. Here’s how it works.

A couple of years ago, Tehama County Sheriff Dave Hencratt needed to reduce his jail population and find ways to reduce recidivism in his community. This was all part of AB 109—Public Safety Realignment—a state law that transferred a significant number of low-risk criminal offenders from state jurisdiction to the counties.
